This Privacy Policy is for www.freeformphysio.ca (our “Website”) which is owned and operated by FreeForm Physio & Pilates Inc. (“FreeForm”, “we”, “us”, or “our”). This Privacy Policy describes the personal information that we collect, use, and disclose when you use our Website or the services that we provide.

The following privacy policy governs the collection, use, and disclosure of information collected from members of the public through the website provided by FreeForm Physio & Pilates Inc., (www.freeformphysio.ca). This policy does not apply to information collected by FreeForm Physio & Pilates by other means.

FreeForm Physio & Pilates’ collection, use, and disclosure of personal information is managed in accordance with the Health Professions Act (HPA) and the Freedom of Information and Protection of Privacy Act (FIPPA). We also abide by the rules and regulations imposed by the College of Physiotherapists of British Columbia with respect to the collection, use, and disclosure of your personal health information.

 By accessing or using our website or our services, you agree to accept this Privacy Policy, including the terms and conditions of this Privacy Policy that relate to the ways that we collect, use, and disclose your personal information.

 What personal information is collected and how is it used?

 When you visit the FreeForm Physio & Pilates website [www.freeformphysio.ca] the web server automatically collects a limited amount of information essential for the operation and security of the website. Some of this information (e.g. browser type) does not identify you, while other information, such as an IP address, may identify you. However, FreeForm does not attempt to link IP addresses with the identity of individuals visiting its website. Information that is exchanged between your computer and our server may be used for analytical purposes.​ While viewing parts of our website, small data files called “cookies” may be stored on your browser to assist you when returning to a specific page or section on our site. If you have concerns about cookies, you can set your web browser settings either not to accept cookies or to provide appropriate alerts.

There are a number of ways in which you may choose to provide FreeForm with personal information. If you send FreeForm an email, or fill out a feedback or request form, you may choose to include information such as your name, email address, and other information specific to your request. Personal information is not collected by FreeForm unless you specifically choose to send it to FreeForm. FreeForm Physio & Pilates takes all reasonable means to protect confidential information you send to it. If you voluntarily communicate personal information or confidential information, within the framework of the use of this website or by email, this information will be accessible only to the personnel of FreeForm Physio & Pilates. We will not divulge any of your personal information to third parties without your consent.

 We collect and maintain different types of personal information regarding the individuals with whom we interact. This personal information may include, but is not limited to:

●      Contact and identification information, such as your name, email address, telephone number, and home address. For example, you will be asked to provide us with this information when you create a client profile with us or book an appointment.

●      Personal health information, including your health card number, patient records, health insurance information, medical charts, appointment history, and other similar health-related information that is relevant to your use of our services.

●      Payment information, including your credit card number and billing address (Please note we use a third party application called the “Jane App” (see below in the next session for more information). Jane stores your credit card information directly on Jane’s servers. When you enter a credit card in Jane, Jane instantly transfers that data to one of Jane’s payment processing partners through encrypted transfer. Jane’s PCI-compliant payment processing partners store that information for Jane. The default behavior of these partners is to store the credit card information so that refunds can be processed. Jane’s partners for payments have been very carefully chosen, and they use the same 128-bit encryption as the big banks around the world. They send Jane back an encrypted key (a token) which represents the credit card so that Jane can continue to bill against that card if the customer wishes, but note that this token can’t be used outside of Jane. The only information that Jane stores about the credit card are the last 4 digits and the expiration date so that the customer will know which card they gave you.)

●      Employment-related information that you choose to provide to us if you are applying for a position with us, such as the personal information included in your resume.

●      Other personal information that you choose to provide to us when you contact us, provide us with feedback, or otherwise communicate with us. The types of personal information that we collect will depend on what personal information you choose to provide to us.

Use of Third Party Application - The Jane App Software

 We use a practice management software called the Jane App. The Jane App is a service provider which supplies data storage and clinic management services through its proprietary platform. Jane collects, uses, and stores your personal information on our behalf in connection with the operation of our business. For example, when you make a booking on our Website, your booking will be facilitated by Jane and the information that you provide will be stored on Jane’s servers on our behalf. We have taken reasonable steps to ensure that Jane will only collect, use, and disclose your personal information based on our instructions in connection with operation of our business and in accordance with applicable laws. Our Website may contain links to other websites. When you access another site, this Privacy Policy and Terms of Use will no longer apply, and you may be subject to any terms of use of that website. We are not responsible for the privacy practices or content of external websites. We cannot control which websites link to our website. Such links do not indicate our endorsement.

 In no event will FreeForm be liable to any party for any direct, indirect, special or other consequential damages for any use of our website, or on any other hyperlinked website, including, without limitation, any loss of programs or other data on your computer or otherwise, even if we are expressly advised of the possibility of such damages.
